ADHD Assessment
Adult ADHD assessment and treatment planning is available at Green Bay Medical Centre for our enrolled patients who are 18 years of age and above.
ADHD assessments are provided by Dr Cushla Borthwick and Dr Ona Blanks.
This service supports adults experiencing ongoing difficulties with attention, concentration, focus, organisation, time management, or impulsivity. An assessment can help determine whether ADHD may be contributing to these challenges.
What to expect
The ADHD assessment includes:
- Screening questionnaires completed prior to your appointment
- A 30-minute GP assessment appointment, including review of symptoms, history, and functioning
- A 15-minute GP appointment to discuss diagnosis and treatment options, if clinically appropriate
The total fee for an ADHD assessment is $350. This includes both the initial assessment and follow-up appointment if required.
Medication may be prescribed if appropriate.
Some patients may require an ECG before starting medication. This can be completed at the practice for an additional fee of $40.
Before your appointment
To begin the process, patients are required to complete:
- ADHD Self-Reporting Symptom Scale
- ADHD Observer Questionnaire
These are reviewed by your GP before your appointment.
Booking an appointment
A $150 deposit is required to secure your booking.
Once your completed questionnaires and deposit have been received, our team will contact you to arrange an appointment with Dr Cushla Borthwick or Dr Ona Blanks, depending on availability.
Completed forms and payment confirmations can be emailed to: nurse@gbmc.co.nz
You may also discuss suitability for an ADHD assessment with your GP.
Important information
- 24 hours’ notice is required for cancellations or changes
- Deposits are non-refundable for missed appointments
- A new deposit is required to rebook missed appointments
